Your Payment Form was Received
You have successfully completed Step Three and submitted your data and signed the authorization form.
- If you successfully submitted all three forms, the next step is for our team to review the information for completeness. Please check the email address for the contact person you listed. If information or a form is missing we will notify you.
- Technology is not perfect. If you have not received a confirmation from us within 4 regular business hours (excluding holidays and weekends), please call us.
- When we have all the information, we will call the main contact to go over final details. Your credit card will not be charged until we take your loved one into our care.
- The Next of Kin (or authorized agent) will be sent the final service agreement to sign online.
- We will send you updates on the status of the process by emails to the contact person you provided.
